Brooklyn Assertive Community Treatment (ACT) Program

The ACT Program is a mobile, team based approach that delivers comprehensive and effective services to participants who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. Program participants are referred to the ACT Team via a single point of access (SPOA) and are all currently residents of Brooklyn counties.

The ACT Team supports participant recovery using a focused approach that provides participants residing in the community with the tools needed to obtain and maintain housing, employment, relationships, and relief from symptoms and medication side effects. The services are developed thorugh the participant-centered service planning process and adjusted through the process of daily team meetings.
