Assertive Community Treatment (ACT) is a mobile, team based program that delivers comprehensive and effective services to participants who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. Program participants are referred to the ACT Team via a single point of access (SPOA) and are currently residents of Brooklyn or in the four Brooklyn Mental Health Shelters currently funded by the NYC shelter system.
ACT supports participant recovery using a focused approach that provides participants, whether residing in the community or residing in shelter, with the tools needed to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects.
