Brooklyn Health Home Supported Housing MRT is designed to help persons living with Severe and Persistent Mental Illnesses (SPMI) who have the desire to obtain permanent housing and the capacity to maintain that housing.

We help individuals integrate into their new community while remaining independent and accessing community resources. Our services are person-centered and focus on reintegration into the community, psychiatric stability, wellness, and attainment of educational and vocational goals.

If you feel that you or a loved one may be a candidate for supportive housing, your first step is to make sure they are eligible. Eligibility requirements for supportive housing differ depending on where someone resides in New York. Most units are designated for single adults who are homeless and need mental health support.
