Children’s Care Coordination works with youth ages from birth to age 21 who primarily have a mental health diagnosis, two or more chronic physical conditions, or have been affected by complex trauma.
We are here to support children who may:
- Go to the Emergency Room or Hospital a lot
- Have behavior issues that impact their daily routine, for example suspended from school
- Have many difficult medical needs
- Missed school a lot when they’re sick or having behavior issues
- Have a recent psychiatric admission
- Have a parent or caregiver who feels stressed or overwhelmed
- Have no supports or difficulty connecting to needed professionals or services

Our Services
Our Care Managers work directly with children to learn all they can about the child/youth and their family’s unique needs. They then create a unique care plan that fits their goals through:
- Comprehensive Care Management where we work with the child and their family on their goals and a plan of care to meet their unique needs.
- Care Coordination and Health Promotion where we talk with everyone who works with the youth to plan a strategy their care, including making appointments and providing follow-up after appointments.
- Comprehensive Transitional Care to help with health care and preventive care needs and, when needed, make smooth transitions when changing medical providers, services, or programs.
- Referrals to Community and Social Supports to introduce you to additional community programs, events, and services that can keep you active, connect you to your community, and develop life skills.
- Additional Family Supports to help make sure the youth and their family have the necessary social, professional, and service supports to meet their goals and ensure caregivers have the tools to manage their child’s health care needs.
Children's Care Coordination provides services to young New Yorkers of all races, ethnicities, religions, gender expressions, and sexual orientations under the age of 21 who have a chronic health, medical, and/or mental health conditions which need a doctor’s care
We help youth who are insured as well as those who are on Medicaid or able to qualify for Medicaid.
