Our intake team will connect you to experienced staff at a program located throughout the five boroughs who will help assess your needs and set up services.

Assertive Community Treatment (ACT)

Our ACT programs are a mobile team-based approach that delivers comprehensive and effective services to participants who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches.

Through our focused approach, we provide individuals living in their communities with the tools needed to find and maintain housing, employment, relationships, and relief from symptoms and medication side effects. These services are developed with the client’s input and are adjusted based on their feedback.

Care Management

Our care management services help adults with chronic illnesses get and use the medical, social and community services they need to stay healthy. Care Coordinators help participants figure out and take the actions needed to get and stay healthy, like making it to appointments, sticking to a medication schedule, and accessing benefits.

Home and Community Based Services (HCBS)

Our HCBS program helps people connect to educational resources and training while empowering and assisting them in obtaining employment.

We offer in-home and community-based sessions and case coordination with other providers, employment and education-based supports, and peer support services.

Personalized Recovery-Oriented Services (PROS)

Our Personalized Recovery-Oriented Services (PROS) programs offers recovery-oriented individual and group support in managing chronic mental illness.

Substance Use Treatment

Our substance recovery programs can help you work toward your unique recovery goals, including getting and staying clean and sober, getting a job, or returning to school.

Services include:

  • Individual and group counseling
  • Skills development
  • Relapse prevention
  • Psychiatric services
  • Medication to reduce cravings and/or address your mental health concerns
  • Nursing exams and referrals to medical doctors
  • Referrals for housing assistance and accompaniment to appointments
  • MetroCards for eligible participants

Who is eligible?

Our services are for adults aged 18 and up living with a chronic mental illness that causes significant functional deficits. We provide services regardless of a person’s ability to pay—this includes the underserved, those with little to no income, and those who are uninsured or on Medicaid. We also provide care to active-duty military personnel and veterans. We accept most major insurance plans.
