Care Coordination addresses the needs of adults living with chronic mental illnesses, health needs, or substance use issues by creating a supportive network to navigate the chaos of multiple providers, locate the resources they need, and develop schedules to stay healthy.
We use a community-based, client-centered approach to help New Yorkers improve the quality of their lives and minimize disruptive events such as hospitalizations. We do this by providing direct support to clients, linking them to necessary care and supports, providing advocacy, and helping to coordinate their care.

To help you create a care plan that fits your life and guides you through the steps to thrive, we provide:
- Comprehensive Care Management where we work with you on your goals and a plan of care to meet your unique needs.
- Care Coordination and Health Promotion where we work together to plan a strategy for care, make appointments, and provide follow-up after appointments.
- Comprehensive Transitional Care to help with health care and preventive care needs and, when needed, make smooth transitions when changing medical providers, services, or programs.
- Referrals to Community and Social Supports to introduce you to additional community programs, events, and services that can keep you active, connect you to your community, and develop life skills.
We serve adults of all races, ethnicities, religions, gender expressions, and sexual orientations who have chronic mental illnesses, health needs (including diabetes, asthma, hypertension, heart disease, and HIV), substance use issues, and cardio-metabolic disorders.
We help everyone regardless of their ability to pay – this includes the underserved, those with little to no income, those who are insured, and those who are uninsured or on Medicaid.
