Our supportive housing programs provide New Yorkers living with mental illness affordable apartments and full access to the services they need to stay housed and well.

Residents are connected to an on-site network of professionals who help them navigate any challenges they might face. We offer transitional and permanent supportive housing options and assist individuals and their families as they work through recovery goals in a safe, stable environment.

ACCESSIBLE

Our intake team will connect you to experienced staff at a program located throughout the five boroughs who will help assess your needs and set up services.

PERSONALIZED

Our care plans are individualized to provide you the best care possible. Our team speaks a wide range of languages, including English, Spanish, Russian, and Haitian Creole.

DIVERSE

We serve people of diverse religions, races, gender identities, sexual orientations, and physical and intellectual abilities, and accept Medicaid and most major insurance plans.

Community Residences/Single Room Occupancy (CR/SRO)

  • Permanent and transitional housing for adult individuals living with psychiatric disabilities.
  • Provides access to rehabilitative services and educational activities.

Intensive Supported Apartment Program (ISAPS)

  • Medicaid billable housing that focuses on restorative services.

Transition Housing

  • Housing for young adults leaving foster care, transitioning from hospital care, and transitioning from Community Residences or ISAPs.

Who is eligible?

If you feel that you or a loved one may be a candidate for supportive housing, your first step is to make sure they are eligible. Eligibility requirements for supportive housing differ depending on where someone resides in New York. Most units are designated for single adults who are homeless and need mental health support.
